Fraser seeks a Community Health Worker to support, educate, and coordinate care for children and families in the areas of medical, behavioral health, pharmacology, community services as well as Fraser services. The ideal candidate will demonstrate compassion and commitment to the clients we serve.

Responsibilities:
This person will work collaboratively with other members of Fraser's Integrated Healthcare team, children and families, and community organizations to provide comprehensive non-clinical care coordination via telehealth. Duties include assisting families in the intake process, supporting the client's individualized holistic health action plan, assessing needs and making appropriate referrals, researching available programs and services and identifying barriers to those resources, and developing and maintaining a caring and trusting relationship with children and families. Accurately documenting services and completing documentation in a timely manner are also an important component of this position.

Schedule:
This is a full-time position working during standard business hours, Monday - Friday.

Required Qualifications:

Applicants can qualify for this position under the following MN Statues:

A peer support specialist as defined in Minnesota Statutes, section 256B.0615
A family peer support specialist as defined in Minnesota Statutes, section 256B.0616
A case management associate as defined in Minnesota Statutes, section 245.462, subdivision 4, paragraph (g) or section 245.4871, subdivision 4, paragraph (j)
A mental health rehabilitation worker as defined in Minnesota Statutes, section 256B.0623, subdivision 5, clause (4)
A community paramedic as defined in Minnesota Statutes, section 144E.28, subdivision 9A
A peer recovery specialist as defined in section 245G.07, subdivision 1, clause (5)
A community health worker as defined in Minnesota Statutes, section 256B.0625, subdivision 49

Additional requirements:

  • Valid Certificate from the Minnesota State Colleges and Universities (MnSCU), demonstrating the completion of an approved community health worker curriculum required.
  • High school diploma, GED, or equivalent, required.
  • Must demonstrate strong competency in navigating systems of care within children's mental health.
  • Must demonstrate the ability to effectively and collaboratively communicate through non-adversarial problem solving, active listening, and positive, constructive interactions.
  • Ability to perform office-related duties as well as carry out responsibilities in home and community-based settings, including the ability to work off-site and travel as needed.
  • Ability to successfully complete MN DHS criminal background check.
  • Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection.
  • Must communicate effectively in the English language, including verbal and written communication with both internal and external contacts.
  • Basic mathematical skills, including the ability to add, subtract, multiply, divide and calculate percentages required.
  • Ability to recognize problems, establish facts, and draw valid conclusions required. Excellent organizational skills and attention to detail required.
  • Physical requirements include frequent lifting and carrying up to 12lbs., and exerting 10-12 lbs. of force regularly.
  • Driving for Fraser business purposes may be required. If required to drive, the following requirements must be met:
    • Possession of a valid Minnesota driver's license and maintenance of a satisfactory driving record consistent with Fraser policy
