Job SummaryThe Ambulatory Population Health team is seeking a community health worker to be a part of the inpatient/outpatient Population Health program. This position will help patients navigate the health care system and provide support for social determinants of health. Through a risk assessment, this individual will identify those patients who need further assistance with their care and/or resources. In the outpatient setting, this position will provide information for at-risk patients regarding medical and psychosocial support services and serve as a liaison between health, social services and the community to facilitate improved health outcomes. This role will involve building relationships with patients through trust, motivational interviewing and empowering patients to take an active part in their health. Individuals will also provide information and resources to clients to improve their health literacy, increase retention in medical care and improve treatment adherence to achieve better health outcomes. Primary patient interactions will occur through home visits, which will aim to facilitate connection to resources, application completion and identification of community support systems. This position will also include community outreach to identify appropriate resources. Coordinator will work in collaboration with multidisciplinary teams to ensure patient has obtained needed resources and health care services, and will maintain patient confidentially and documentation in electronic health record and database.
