Main Duties and Responsibilities:
Work as a team, in concert with each individual to develop an Individual Treatment Plan that identifies the services and supports that will enable each person to live as independently in the community as possible. Services and supports may include activities such as assisting with skill development in the following areas: symptom management, budgeting, housing, health and wellness, transportation, relationship building, medication monitoring, and obtaining or maintaining benefits
Provide individual and/or group psychotherapy to address specific symptoms and behaviors as specified in the individual's treatment plan. Interventions include empirically supported psychotherapies, such as, cognitive behavioral therapy for psychosis, behavioral activation for depression, dialectical behavior therapy, acceptance and commitment therapy, and motivational interviewing strategies.
To the extent requested by the individual, provide collaboration, consultation, and education to significant others and family members who are interested in assisting the individual in their recovery process.Treat all individuals with dignity and respect, providing services that are respectful of diagnosis, age, gender, and culture.
Assure that all changes in the individual's status or needs are communicated promptly to the team and documented to enhance continuity of care.Assure that services are provided as developed by the individual and team as documented in the Individual Treatment Plan.In case of illness or crisis, provide coverage for team members to assure and maintain continuity of care for each person served.
Participate in the ACT 24-hour on call rotation schedule, daily team meetings, and scheduled trainings.

Minimum Education and Experience:
Master's degree in social work, psychology, or human services related field and two years of experience in related positions; at least one year includes experience in community based mental health with adults recovering from mental illness or combination of equivalent experience and training.
Licensed as a mental health professional in the State of Minnesota preferred. Those who are not licensed but are eligible for licensure and are otherwise qualified may also fulfill this role, as long as full mental health professional licensure is obtained within 24 months of assuming the role.
